Describe the bug
Pressing the 'Clear form' button clears everything except the Warning label.

To Reproduce
Steps to reproduce the behavior:

  1. Activate one of the multi-something radio buttons
  2. Click on Clear form button
  3. Warning persists

Expected behavior

All items should be cleared/reset to a fresh state

Please complete the following information:

  • version 1.2.0
  • Does not affect record in data base
