Nitro v2 / v3

Reproduction

Describe the bug

$production is not working in environments like Cloudflare Pages because NODE_ENV is not set by Nitro (or possibly removed by Cloudflare?).

In my reproduction repo, /dev should not be available when deployed to Cloudflare Pages.
