One way of training raw speed, is to do the shortest tests on Monkeytype. 15 s or 10 word tests are examples. Many do this regularly.
Prepare yourself mentally, then blaze out of the gates as fast as you can while trying to get everything correct too. Gogogo!

I prefer to train word bursting. This is the method suggested by Sophie above: From a state of mental preparedness, type each word as a minimum number of actions, fast and true. You may pause between words as needed while training, the focus is on the execution of each word.

Some get frustrated when trying this. If you find yourself failing repeatedly, try to take a breath and refocus for each attempt. If that isn't enough, start out by going as slowly as you need to type "in the blue" (low speed but no mistakes) over and over until you're ready to gradually speed up.

Reading books by copytyping them in is a great way of racking up mileage in a realistic setting while keeping it interesting. The bare-bones way of simply using a physical book and a blank document works fine, and has been used by such masters as Sean Wrona and Viper, but I like to use tools that can keep track of progress, speed and accuracy statistics and more. Nowadays I mostly do my daily rounds on EnterTrained. Earlier I did most of it offline with the Amphetype program.

I think you can enjoy type-reading from, say, around 40 WPM typing speed. Focus on accuracy over speed. You want to build up muscle memory for typing words and common n-grams correctly. I sometimes retype a word a couple of times if I get it wrong. I often read the next paragraph before typing it in, to prepare myself mentally.

Typically, I'd get a book as a text file from Project Gutenberg or similar, and import it into Amphetype. Experiment a little with how large text chunks you want to use; I use somewhere between 300 and 900 characters. I've "read" Machiavelli that way, as well as Alice Through The Looking Glass, some Lovecraft and a 19th century book of questionable moral value!

For type-reading books, I recommend not using too strict parameters so you won't have to type the same page again very often. Your average text speed minus 10–20 WPM and 96–97% accuracy is a good starting point. With Amphetype, you set the limits on the Sources tab, while selecting what text/lesson to type next. I turn off the need to press SPACE before each lesson, so I can type fluently. The timer doesn't start until you start typing a page, so take your time before diving in if you want to.

I've already mentioned two of my favorites, EnterTrained and Amphetype.
One neat offline alternative is CopyType, a dedicated e-book typer program.

Furthermore, you can let Amphetype generate a list of your "most damaging" (or slowest, most mistyped etc.) words or trigrams, and send them to the Lesson Generator! I recommend editing out rare words before making the lessons themselves so you don't risk training words like 'Cthulhu' and 'Shub-Niggurath' a lot, but otherwise this is very useful once you've racked up some mileage in your database.

Here's an example of pangram training using MonkeyType. I used the common "The quick brown fox..." phrase which comes up by default on the custom test, pressed "change" then "random" and a suitable word count. I'd use that until I felt ready to move on to the usual word list tests.
