Add Headless Support to UI

This update is intended to support the scenario where the app form may be embedded in another app and does not need to display any top nav or side bar. This implementation will toggle and apply styling based on the following querystring parameter: headless=true

Tried it on vercel preview (https://jhub-apps-qm156rtja-nebari-jhub-apps.vercel.app/services/japps/create-app?headless=true) - looks great to me!

Something we may want to consider later is whether the form should be centered or stay left-aligned but get stretched when embedded in an iframe.
